Borrowed Resource Request Form

This members-only form is used for approval to borrow resources only, not use of the facility. It does not require any payments or deposits. Once we have received your request, a member of our Facilities Team will let you know if your request can be accommodated. Requestors must be over the age of 18. *Please note: All borrowed resources must be picked up and returned during our normal office hours, excluding office holidays. (M-F 8:30am-5:00pm) No Sunday pickups or dropoffs.
